Anatomical vs. round implants
Anatomical (teardrop)
Mimic the natural breast: more volume in the lower pole, less in the upper. Best for patients with little chest fat, small native breasts, thin chest wall and a conservative result. Rotation is rare with new-generation textured shells.
Round implants
Distribute volume evenly. Best for patients seeking pronounced upper-pole projection — the “lifted décolletage” look. Also preferred in revision surgery and mastopexy with augmentation.
Implant profiles
- Moderate profile: wide base, modest projection. For wide chests.
- High profile: balance between base and projection. The most requested option in Medellín.
- Extra-high profile: narrow base, maximum projection. For narrow chests seeking pronounced volume.
Pocket placement
Subglandular (over the muscle)
Faster, less painful recovery. Risk: implant feels and looks more in patients with little breast tissue. Higher risk of visible borders.
Submuscular (under pectoralis)
Muscle covers the implant — improves naturalness in thin patients. Recovery is more painful in the first 7 days. Preferred when there’s little glandular tissue.
Dual plane
Hybrid technique: upper pole submuscular, lower pole subglandular. The most chosen approach in Medellín — combines advantages of both planes.

Real risks to know
- Capsular contracture: 1–3% with submuscular textured implants. Treatable.
- Residual asymmetry: no body is 100% symmetric. We minimize, not eliminate.
- Nipple sensitivity loss: 5–10% transient, < 1% permanent.
- BIA-ALCL: rare lymphoma associated with macrotextured implants (we haven’t used that texture since 2019).
