What is a BBL exactly?
Brazilian Butt Lift = liposuction + fat transfer to the buttocks, done in one surgical act:
- Liposuction of donor areas (back, flanks, abdomen, arms).
- Fat is gently processed by decantation or centrifugation.
- Injection into the buttocks layer-by-layer using rigid cannulas, only in the subcutaneous plane.
The “Brazilian” name comes from pioneering work by Brazilian surgeons in the 1990s, but today’s procedure is dramatically safer.
The MIS-BBL safety protocol
Until 2018, BBL had the highest mortality among cosmetic surgeries (1 in 3,000 cases per ASPS data). Cause: intramuscular injection caused fat embolism through deep gluteal veins. The MIS-BBL protocol sets three absolute rules:
- Zero intramuscular injection. Subcutaneous plane only. This is the most important rule.
- Cannulas of at least 4 mm, rigid (not flexible).
- Constant cannula motion during injection — never static.
If your surgeon doesn’t mention MIS protocol or doesn’t use ultrasound guidance, look for another option.
Am I a candidate?
- Adequate donor fat: minimum 1.5–2 liters available across all areas.
- BMI between 22 and 30. Very thin patients lack donor fat; very heavy patients may need a two-stage approach.
- Good gluteal skin quality without extensive stretch marks.
- No history of thrombosis or coagulation disorders.
- Willingness to respect prone position for the first month (sleeping face-down).

Range: USD 3,200 – 5,000. Factors that drive cost:
- Number of liposuctioned areas (more areas, more OR time).
- Volume of transferred fat.
- Add-on procedures (simultaneous mastopexy or abdominoplasty).
- Type of OR and anesthesia (always third-level facility with ICU).
Realistic recovery timeline
- Days 1–14: no direct sitting on buttocks. Sleep prone or sideways. U-shaped cushion if sitting is unavoidable.
- Day 7: first follow-up, gentle manual lymphatic drainage.
- Week 2: office work resumed with special cushion.
- Month 1: normal sitting allowed. Continue manual drainage.
- Month 3: preliminary result visible. Reabsorbed fat is gone.
- Month 6: final result. Surviving fat stays permanently if weight remains stable.
Real risks to know
- Asymmetry: body reabsorbs fat unevenly. 5–10% of patients want a touch-up at 12 months.
- Seromas: 3–5%, drained in office.
- Fat necrosis: firm nodules that may need removal.
- Fat embolism: < 0.01% with strict MIS-BBL protocol.
